In fairness, I've never seen literal hardcore porn there. I've seen stuff that goes way beyond what the ToS says, but either they do actually take down the hardcore stuff, or it's pretty well hidden.

I presume the 'no images of self harm' is to avoid people posting photos of cuts/etc they've inflicted on themselves and encouraging others to do so. Which seems weird, but I've heard of it happening...
